Your booking will be
made through your doctor's surgery. For a smooth admission
process, we ask that you attend to the following matters prior
to your admission:

Our Pre Admission
Office is open from 8:30am to 6:00pm, and it is preferable
that you attend in person to complete or deliver the necessary
paper work. If you are unable to do so, please telephone our
Pre Admission Coordinator with your details on 9788 3466.
This contact you have with the Pre Admissions Office will
confirm your details, including date and proposed time of
your admission, your procedure and your expectations relating
to discharge and post discharge care.
Private
Health Insurance
Please contact your
Private Health Fund to confirm your level of cover and whether
you have an excess or co-payment on your policy. In the event
that you do have an excess or co-payment, your excess is payable
at the time of your admission, and co-payment(s) are payable
at discharge.
What to bring
- Name and details of your General Practitioner
- Details of Insurance Cover
- Health Fund membership details
- Work Cover/TAC Details
- Department of Veterans' Affairs card
- Pension Entitlement card
- Pharmaceutical Entitlement card
- Relevant existing X-rays and CT scans
- All medications you are currently taking in their original
container (including tablets, vitamins, mixtures, creams,
inhalers or drops). If possible, please bring a list from
your General Practitioner of your current medications, their
strengths and dosages.
- Sleepwear, dressing gown for overnight stay and slippers
or comfortable shoes
- Glasses, hearing aids and non-electronic walking aids
- Toiletries, including soap, toothpaste and toothbrush
- Reading material
For children please
consider
- Bottles
- Nappies
- Books
- Toys or Games
You will be provided
with a security locker for medication and small personal items.
However, we recommend that you do not bring cash amounts greater
than $50 or any jewellery items with you. A few dollars is
recommended for incidental items such as newspapers or articles
from the Pharmacy or Kiosk.
The Hospital cannot
accept responsibility for your valuables.
Mobile phones should not be used in the Wards as their
signals can interfere with electronic medical equipment.
